WISD ANNOUNCES KINDERGARTEN REGISTRATION

9:00-11:00 a.m. or 6:00-7:30 p.m.
Registration for Kindergarten for the 2011-2012 school year will be held on Thursday, April 14, 2011, from 9:00-11:00 a.m. and also from 6:00–7:30 p.m. Parents/Guardians need to bring the child's birth certificate, immunization record, and proof of residency in the WISD with them in order to register their child in kindergarten.
Kindergarten students must be five years old by September 1, 2011. The parents/guardians of the student must be residents of WeatherfordIndependent School District.

NOTE: For information on the elementary school that serves your address call 817-598-2803 or go towww.weatherfordisd.com and click on “Campus Locator.” You will then be able to type in your address and submit for the correct school information.

Kindergarten classes are full day classes for the 2011-2012 school year.
Parker County Hospital District will be open for immunizations from 8:00 A.M.—12:00 and 1:00-4:00 P.M. on April 14 at 1130 Pecan Street. In addition, the Texas Department of State Health Services provides vaccine clinics at Weatherford Community Center at 701 Narrow Street. WCC clinic times are 1st and 3rd Thursday of each month from 9:00-11:00 A.M. and 1:00-3:00 P.M. Vaccines may also be obtained from private care health providers during annual pediatric exams.
Please bring immunization records and Medicaid card if available.
IMPORTANT NOTICE: THERE WILL BE NO GRACE PERIOD FOR VACCINES IN THE 2011-2012 SCHOOL YEAR. CURRENT VACCINE RECORDS WILL BE REQUIRED FOR THE STUDENT TO ATTEND THE FIRST DAY OF CLASS.
THERE WILL BE NO EXCEPTIONS!
2011-2012 Kindergarten Registration Requirements
In order to register your child for Kindergarten for the 2011-2012 school year:
· Your child must be 5 years old on or before September 1, 2011.
· You must provide an original copy of the child’s birth certificate.
· A copy of the child’s social security card is requested.
· You must provide proof of residence (something showing current service into the home with the parent’s name) such as a utility bill. A lease agreement or closing document is acceptable, provided the document includes the name of the person who is enrolling the child.
· Parent or guardian must present identification (driver’s license).
· You must provide a copy of the child’s immunizations. *
*A student must be fully immunized against certain diseases or must present a certificate or statement that, for medical reasons or reasons of conscience, including a religious belief, the student will not be immunized. For exemptions based on reasons of conscience, only official forms issued by the Texas Department of State Health Services (DSHS), Immunization Branch, can be honored by the District. This form may be obtained by writing the DSHS Immunization Branch (MC 1946), P.O. Box 149347, Austin, Texas 78714-9347; or online at https://webds.dshs.state.tx.us/immco/affidavit.shtm. If the parent is seeking an exemption for more than one student in the family, a separate form must be provided for each student.
The immunizations required are: diphtheria, rubeola (measles), rubella (German measles), mumps, tetanus, pertussis, poliomyelitis (polio), hepatitis A, hepatitis B, and varicella (chicken pox). The school nurse can provide information on age-appropriate doses or on an acceptable physician-validated history of illness required by the Department of State Health Services. Proof of immunization may be established by personal records from a licensed physician or public health clinic with a signature or rubber-stamp validation.
If a student should not be immunized for medical reasons, the student or parent must present a certificate signed by a U.S. licensed physician stating that, in the doctor’s opinion, the immunization required poses a significant risk to the health and well-being of the student or member of the student’s family or household. This certificate must be renewed yearly unless the physician specifies a life-long condition.
