Chronic Disease Self-Management Program

Chronic disease self-management classes will be offered from 10 a.m. to 12-30 p.m. on Mondays July 8 through Aug. 12 at the Parker County Extension Office, 604 North Main Street in Weatherford.
The Chronic Disease Self-Management Program is a nationally-recognized program that was developed at Stanford University. The 6-weeks evidence-based program helps people living with long-term health concerns and chronic diseases such as diabetes, heart disease, long-term pain, breathing problems, depression, arthritis, and more.
The class will cover topics on managing symptoms, healthy eating, medication usage, stress management, making informed treatment decisions, and working with your health care provider.
